Job summary

Lutheran Social Service of Minnesota is seeking a Care Coordinator to assess, plan, and coordinate services for enrollees, delivering culturally appropriate, person-centered care in a dynamic team environment.

The role requires Minnesota licensure (LSW or RN), strong communication skills, and the ability to work independently while engaging with diverse populations. A driving license and background check are required.

Qualifications

• Licensed Social Worker or Registered Nurse licensed in Minnesota.
• Minimum of one year of assessments experience and knowledge of community resources.
• Valid driver's license.
• Must pass background study and motor vehicle check.
• Strong written and verbal communication skills.
• Basic computer skills; Microsoft Office Suite.

Responsibilities

• Conduct initial and annual comprehensive assessments for enrollees; create an individualized Plan of Care.
• Coordinate provision of services and ensure access to care.
• Provide culturally appropriate, person-centered care and education to enrollees.
• Promote knowledge of resources, strengths, and supports while ensuring fiscal and professional accountability.
