Job summary

ADVENT seeks a Care Coordinator in South Nashville to connect patients, providers, and teams and guide them through their treatment journey. You will support patients in understanding options, navigating insurance, and coordinating care across clinic and virtual settings.

The role emphasizes empathy, problem-solving, and communication, with opportunities to train at multiple clinics in Middle Tennessee. 32-hour flex full-time position with weekday hours.

Qualifications

• Minimum of 2 years' experience in healthcare coordination or a similar patient-facing role.
• Bachelor’s degree in healthcare or related field or equivalent experience.
• Experience guiding patients toward treatment goals and outcomes.

Responsibilities

• Serve as the primary point of contact for patients and families, building trust and rapport.
• Identify and resolve care barriers, guiding patients toward successful completion of their care journey.
• Balance compassion with efficiency, ensuring patients feel supported while protecting clinic workflow.
• Advocate for patients on financial matters, including insurance and out-of-pocket costs.
• Own the patient journey end-to-end with strong listening and communication skills.
• Coordinate scheduling, follow-ups, procedures and documentation with staff and surgeons.
• Monitor patient adherence and engage patients to ensure treatment outcomes.
