Privia Health, LLC is seeking a Health Advisor to provide care coordination for at-risk patients. You will conduct inbound and outbound telephonic support, identify gaps in care, and promote patient outcomes through direct outreach.

Responsibilities include assisting physician practices with quality care goals, collaborating with the care team, and making referrals to internal and external resources. A hybrid schedule is required (3 days remote, 2 days in-office).

Competenze

• Prior experience navigating Electronic Medical Record systems (e.g., Epic, Athena, or similar platforms).
• Experience with patient outreach across diverse demographics (pediatrics to adults).
• Hybrid work schedule experience (3 days remote, 2 days in-office).
• Comfortable with new platforms, payer reports, and in-house digital tools.
• Ability to evaluate varied patient cases and know when to elevate to RN.

• Assist physician practices in identifying and completing quality care goals via patient outreach.
• Collaborate with care team to support care coordination for patients.
• Reach out to non-compliance lists to ensure timely disease-specific and wellness visits.
• Handle inbound calls about population health and care coordination.
• Refer patients to internal care team, community resources, or disease management programs.
• Develop relationships with care coordination teams and schedule PCP or specialist appointments.
• Route clinical calls to Nurses, pharmacists, social workers, or other resources.
