Qualifications

Responsibilities

• Navigate patients through ART care plans following navigation procedures.
• Educate patients on ART, HIV basics, HIV transmission and U=U.
• Document progress notes and services in patient charts promptly.
• Refer patients to Insurance or Financial Aid when needed and inform about available resources.
• Participate in weekly ART team meetings and provider meetings.
• Ensure regular contact with patients through communications and prescription management teams.
• Oversee transportation for patients to appointments and access to key services.
• Engage in ongoing HIV-related education and cultural humility training.
• Report hours accurately and follow Diversity Health policies.
