Job summary

Twin Cities Pain Clinic in Edina, MN is seeking a Care Coordinator to serve as the first contact for patients through the care journey. You will manage scheduling, pre-procedure preparation, medical record collection, and communications to support the surgery center and care teams.

Collaborate with providers to align treatment plans, coordinate with external facilities, and educate patients with forms and instructions.

Qualifications

• High school diploma or GED with at least one year of medical clinic experience.
• Associate’s or Bachelor’s degree preferred, plus customer service experience.

Responsibilities

• Schedules surgeries, procedures, and related appointments; serves as the primary contact for scheduling.
• Coordinates with providers and care teams on treatment plans and patient education materials.
• Assists patients with required forms and documentation.
• Performs outreach to patients, providers, and outside facilities to support coordination.
• Maintains patient tracking systems and coordinates follow-up activities.
• Transmits and coordinates medical records and necessary documentation.
• Handles pre-operative information packets and instructions for patients.
• Complies with HIPAA and organizational policies.
• Performs other duties as assigned.
