Job summary

Glenwood is a private nonprofit organization dedicated to supporting individuals with autism spectrum disorders and dual diagnoses. We seek a Care Coordinator to provide therapy and case management within our Dually Diagnosed Program.

The role involves coordinating home visits, managing clinical records, and participating in multidisciplinary meetings. A Master’s in social work and licensure eligibility are preferred, with 3 years of clinical experience.

Qualifications

• Master’s degree in social work or a related field.
• Licensed or license-eligible: LCSW or equivalent per Alabama requirements.
• Minimum of 3 years’ experience in clinical/mental health settings preferred.

Responsibilities

• Provide therapy and case management services for assigned caseloads, including individual, group, and family therapy.
• Coordinate home visits and align services with guardians.
• Manage clinical records and treatment planning per agency standards.
• Handle intake, discharge planning, and referrals; participate in team meetings and reviews.
• Share on-call duties and provide staff training as needed.
