COMHAR is a nonprofit community based organization in Philadelphia seeking a Care Coordinator to support the Intensive Behavioral Health Services (IBHS) program for children. The role coordinates services, collaborates with families, schools, and providers, and guides families through intake, evaluations, and referrals to appropriate programs.

Job duties include meeting families for intake, scheduling evaluations, providing information about medications and services, attending biopsychosocial

Competenze

• Minimum of a high school diploma with 12 college credits in behavioral health/human services.
• Bachelor’s degree in social work, counseling, psychology, sociology, gerontology, counseling education or related degree.
• At least 1 year experience in health or human service field (behavioral health or substance use field preferred).
• Driver's license required.

Mansioni

• Meet with families for intake.
• Coordinate services and schedule/confirm evaluations.
• Provide families with information for medications, outpatient services, neurology, etc.
• Attend biopsychosocial evaluations (CBE/CBR).
• Arrange referrals and placement to Partial Hospitalization Programs, Residential Programs, and inpatient care.
• Home visits as needed.
