Job summary

CHILDRENS CRISIS TREATMENT CENTER INC is seeking a SBS/SBP Care Coordinator in Philadelphia, PA to manage health and proactive relationships with children, families, and school communities. You will coordinate IBHS services, ensure CBH eligibility, and maintain care plans within Carelogic.

The role requires a bachelor’s degree in a relevant field and at least one year of case management or counseling experience with youth and families.

Qualifications

• Bachelor’s degree required in social work or related field, from an accredited institution.
• Minimum of one year of case management, counseling, or direct mental health care experience with children and families.

Responsibilities

• Assess service needs based on a Physician/Psychologist evaluation by completing the IBHS intake.
• Identify, link, and coordinate services including preparation and distribution of IBHS packets.
• Ensure children are CBH eligible to participate in IBHS.
• Facilitate access to services.
• Monitor the effectiveness of services and reassess needs.
• Maintain documentation of care coordination/case management in the Carelogic EHR.
• Meet with client and responsible parent within 1 week of initial contact and maintain weekly follow-up.
• Coordinate with County MH/MR, Children and Youth, schools, day care, and other child-serving systems to ensure continuity of care.
• Work with the clinical treatment team to ensure coordinated services.
• Enter outcomes data (CANS, Arizona) into Carelogic.
• Assume other duties assigned by Clinical Supervisor/Lead Clinician and Team Leader.
