Job summary

COMHAR, Inc. is seeking a Care Coordinator for the IBHS program in the Children Services department. The role coordinates services for children, families, and community partners to support intensive behavioral health services.

The position emphasizes collaboration with families, schools, and providers, with responsibilities including intake, scheduling, and referrals. A minimum education and licensure requirement applies, and bilingual abilities are a plus.

Qualifications

• Minimum of a high school diploma with 12 college credits in behavioral health/human services.
• Bachelor’s degree in social work, counseling, psychology, sociology, gerontology, counseling education or related degree.
• At least 1-year experience in the health or human service field (behavioral health or substance use field preferred).
• Driver's license required.

Responsibilities

• Meet with families for intake.
• Coordinate and schedule evaluations/re-evaluations.
• Assist families with information for medications, outpatient, neurology, etc.
• Attend evaluations (Biopsychosocial Evaluation/Re-evaluation).
• Arrange referrals and placement to Partial Hospitalization Programs, Residential Programs, and inpatient services.
• Collaborate with DHS STS Programs.
• Complete packets to be submitted to Community Behavioral Health.
• Respond to CBH responses and follow up on issues.
• Request addenda as needed.
• Conduct home visits as needed.
