Job summary

CBH in Philadelphia seeks a Behavioral Health Liaison to support Clinical Care Managers in procuring behavioral health services for CBH members. The role ensures services are delivered as authorized and aligned with clinical needs.

Key responsibilities include coordinating referrals, supporting ancillary services, and facilitating communication across CBH teams, DBH, Family Court, and school programs. Strong organizational and HIPAA compliance are essential.

Qualifications

• Bachelor’s degree in Social Work or related field.
• PA Child Abuse Clearance required.
• Minimum of 2 years of direct service experience including assessment, counseling and coordinating services.

Responsibilities

• Coordinate referrals: track status, notify providers, ensure initiation of services.
• Make recommendations and referrals for ancillary and supplemental services for all ages.
• Coordinate information between CBH clinical team and DBH, Family Court, and school programs.
• Provide CBH Member Services with referrals lists and maintain post-discharge follow-up to ensure adherence to treatment.
• Facilitate and participate in interagency meetings.
