Job summary

Humana Inc. is seeking compassionate, organized case managers to serve Wisconsin Family Care members. You will assess health and safety needs, develop an MCP, coordinate with a Field Care Nurse, and connect members with social resources.

Most work occurs in or near members' homes, with travel up to 40%. A Bachelor's degree in human services or related field is required (or a Bachelor's in another field with at least 3 years of relevant experience).

Qualifications

• Bachelor's degree in human services or related field with at least 1 year experience serving frail elders or adults with disabilities, or Bachelor's in another field with at least 3 years of relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services addressing health and safety needs in the MCP, in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and every six months, typically at the member's residence.
• Conduct quarterly in-person visits and maintain monthly contact with members by phone.
• Arrange non-medical support services including housing, social integration, and community resources.
• Ensure cost-effective service delivery and maintain accurate MCP documentation.
