Job summary

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) seeks a Bilingual Care Coordinator to join the Milwaukee County CLTS team. The role requires fluency in Spanish and English to help families assess needs, coordinate services, and create individualized care plans that keep children safely in home and community settings.

The position is full-time, Monday–Friday, with a hybrid schedule offering up to 2 remote days per week. Travel across Milwaukee County is expected.

Qualifications

• Bachelor's degree in a human services-related field.
• At least one year of experience working with children or individuals with developmental, physical, or behavioral health needs.
• Fluent in Spanish and English (required).
• Valid driver's license, reliable transportation, and ability to travel throughout Milwaukee County.
• Strong communication, documentation, organizational, and computer skills.

Responsibilities

• Complete assessments to identify client and family needs.
• Develop and implement person-centered service plans in collaboration with families, support teams, and providers.
• Coordinate and monitor services to ensure goals and outcomes are met.
• Facilitate family and team meetings.
• Maintain accurate documentation and client records.
• Connect families with community resources and supports.
• Provide services utilizing trauma-informed and family-centered practices.
• Collaborate with service providers, community partners, and internal teams.
