Job summary

Lutheran Social Service of Minnesota is seeking a Care Coordinator to perform comprehensive assessments and develop individualized Plans of Care for enrollees.

You will coordinate services, promote health education, and ensure culturally appropriate, person-centered care while maintaining fiscal and regulatory accountability. The role requires collaboration with a multidisciplinary team and strong communication, organization, and independence in a dynamic setting.

Qualifications

• Licensed Social Worker or RN licensed in Minnesota.
• Minimum of one year experience with assessments and knowledge of community resources.
• Valid driver's license.
• Must pass background study and motor vehicle check.
• Strong written and verbal communication.
• Proficient in Microsoft Office.
• Detail oriented, organized, analytical and problem-solving.
• Able to work independently and in a team.
• Able to implement strategies to serve culturally diverse populations.
• Able to adapt to a dynamic environment.

Responsibilities

• Conduct initial and annual comprehensive assessments for enrollees.
• Create an individualized Plan of Care based on assessments.
• Coordinate provision of services and monitor progress.
• Provide culturally appropriate, person-centered care.
• Promote enrollee knowledge, resources, and self-advocacy.
• Ensure fiscal and professional accountability in compliance with laws.
