Job summary

Lutheran Social Service of Minnesota is seeking a Care Coordinator in Minnesota to conduct initial and annual comprehensive assessments, develop individualized Plans of Care, and coordinate service delivery with culturally responsive, person-centered care.

Qualifications

• Licensed Social Worker or Registered Nurse licensed to practice in Minnesota.
• Minimum of one year of experience with assessments and knowledge of community resources.
• Valid driver's license.
• Background check and motor vehicle check required.
• Strong written and verbal communication skills.
• Ability to work independently and in a team.
• Cultural competence and ability to serve diverse populations.
• Proficiency with Microsoft Office Suite.

Responsibilities

• Conduct initial and annual comprehensive assessments to enrollees and create individualized Plan of Care.
• Coordinate provision of services and ensure culturally appropriate person-centered care.
