Job summary

Pro Care by Abound Health in Southfield, MI seeks a compassionate Care Coordinator to help individuals receiving Michigan Waiver Services live more independently and stay connected to needed support. Reporting to the Program Director, you will lead direct care staff and build meaningful relationships with clients and families.

You will coordinate personalized services, develop goals in OnTarget, and ensure records meet waiver requirements.

Qualifications

Must have strong communication, organization, and problem-solving abilities.
Experience in care coordination, case management, social services, healthcare, or related field.
Knowledge of IPOS implementation and Michigan Waiver Services is preferred.

Responsibilities

Build trusted relationships with clients, families, direct care staff, and community partners.
Coordinate individualized services that support each client’s goals, independence, and quality of life.
Supervise, coach, and support direct care staff.
Develop client goals and interventions and maintain them in OnTarget.
Coordinate staff schedules and backup coverage for consistent support.
Connect clients and families with community services and resources.
Maintain accurate records and ensure services meet Michigan waiver, IPOS, and Pro Care requirements.
