Job summary

Pro Care by Abound Health in Southfield, Michigan seeks a compassionate Care Coordinator to help individuals with Michigan Waiver Services live more independently and stay connected to support.

Reporting to the Program Director, you will lead direct care professionals, coordinate individualized services, and build meaningful relationships with clients, families, and community partners to ensure quality of life.

This role offers opportunities to impact families daily and grow within a growing team.

Qualifications

• High school diploma or GED required.
• Experience in care coordination, case management, social services, healthcare, or a related field.
• Strong communication, organization, and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of IPOS implementation and Michigan Waiver Services is preferred.
• Proficiency in Microsoft Office; OnTarget experience is a plus.
• Valid driver’s license, reliable transportation, and the ability to pass all required background checks.

Reporting to the Program Director, you will help individuals receiving Michigan Waiver Services live more independently and remain connected to the support they need. Compensation & Benefits

Base pay is determined by factors such as work location, experience, job-related skills, and relevant education or training. In addition to base pay, total compensation may include various other forms of pay. We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits, which begin on your 60th day of employment.
