Job summary

Pro Care by Abound Health in Southfield, MI is seeking a compassionate Care Coordinator to help clients live more independently and stay connected to support services.

You will supervise direct care staff, develop client goals, and coordinate services to improve quality of life. A high school diploma is required, with preferred experience in Michigan Waiver Services and IPOS implementation.

Qualifications

• High school diploma or GED required.
• Experience in care coordination, case management, social services, healthcare, or related field preferred.
• Strong communication, organization, and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of IPOS implementation and Michigan Waiver Services preferred.
• Proficiency in Microsoft Office; OnTarget experience is a plus.
• Valid driver’s license and reliable transportation.

Pro Care by Abound Health is seeking a compassionate and organized Care Coordinator to join our growing team in Southfield, Michigan. Reporting to the Program Director, you will help individuals receiving Michigan Waiver Services live more independently and remain connected to the support they need. This is an opportunity to build meaningful relationships, lead direct care professionals, and make a visible difference for clients and families every day.

Compensation & Benefits:

Base pay is determined by factors such as work location, experience, job-related skills, and relevant education or training. In addition to base pay, total compensation may include various other forms of pay. We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits, which begin on your 60th day of employment.
