Job summary

Center for Family Services in Camden, NJ, is seeking a Care Coordinator/Data Specialist to support CCRT operations through care coordination, data management, documentation oversight, and performance monitoring.

Under supervision, you will coordinate referrals, track 30-, 60-, and 90-day follow-ups, maintain records, and collaborate with clinicians and community partners to promote continuity of care and recovery for participants.

Qualifications

• Bachelor's degree in related field required.
• Minimum 2 years in behavioral health or care coordination preferred.
• Experience with EHR systems and data management preferred.
• Knowledge of HIPAA and confidentiality regulations.
• Excellent communication and time management skills.

Responsibilities

• Coordinate referrals and warm handoffs to services.
• Monitor 30-, 60-, and 90-day follow-ups for engagement.
• Maintain accurate documentation and participant records.
• Collaborate with clinicians and community providers to coordinate services.
• Monitor program data and outcomes.
• Prepare routine reports and assist with QA.
