Job summary

Ncaarbh is seeking a Care Coordinator in Cherry Hill, NJ to manage intake, treatment engagement and utilization review. You will coordinate referrals, review consents, arrange transportation, and follow up with clients to ensure timely appointments.

The role emphasizes coordination across services, collaboration with ICM when needed, and presenting cases at statewide consortia. A master's degree and relevant licensure are required, with 2+ years in co-occurring disorders.

Responsibilities

• Make introductory calls to newly transferred clients to explain their role and review the treatment plan.
• Review initial signed consent forms to ensure completion and upload.
• Coordinate new treatment referrals and obtain releases.
• Ensure intake processes including transportation are in place.
• Complete reminder and follow-up calls about intake appointments.
• Conduct utilization reviews and Continuing Service Reviews (CSR).
• Present cases at statewide DCP&P consortia and be prepared to present.
• May initiate referrals for Intensive Case Management (ICM).
• Send outreach letters to clients who have left treatment.

Job description

The primary role of the Care Coordinator (CC) is to coordinate and monitor treatment engagement and utilization review.

• Making introductory calls to newly transferred clients to explain their role and review the treatment plan.
• Review initial signed consent forms to ensure they are completed and uploaded.
• Coordinate new treatment referrals, if needed, including obtaining new releases.
• Ensure all aspects of the treatment intake are in place, including transportation.
• Complete reminder and follow up calls to clients about their intake appointments. Complete all aspects of utilization review including regularly scheduled
• Continuing Service Reviews (CSR) to determine the next steps for the case including ongoing management at the same level of care, a different level of care or case closure.
• Present cases at the statewide DCP&P consortia, CC will all be trained to present at all statewide DCP&P consortia.
• May initiate referrals for Intensive Case Management (ICM) and collaborate with ICM staff.
• Send outreach letters to clients who have left treatment.

A Master’s Degree in counseling, social work, or related field with an LAC/LPC, LCADC, or LSW/LCSW is required. Must have a minimum of two years of co-occurring disorder experience and applied knowledge of The ASAM Criteria, 3rd Ed., and the DSM-5-TR.

Salary: $61,000.00 – $67,000.00 annually
