Job summary

Location: Camden County, NJ

Job Type/Status: Full Time (Monday through Friday 11:00am-8:00pm)

Salary: $65,000 to $70,000 per year

Position Summary

The Care Coordinator is a Master's-level clinician (LSW) who provides care coordination and clinical oversight of post‑crisis services for individuals served by CCRT. The role ensures continuity of care following crisis intervention, including assessment, service linkage, follow‑up, and supervision of Peer Specialists. Working under the direction of the Lead Clinician (LCSW), the Care Coordinator supports stabilization and recovery by coordinating services post‑crisis and collaborating across community providers and systems of care.

Duties and Responsibilities
Clinical Coordination & Oversight
  • Conduct brief assessments and support care planning following crisis response
  • Provide clinical guidance and oversight to Peer Specialists
  • Ensure services align with trauma‑informed and recovery‑oriented practices
Service Linkage & Follow‑Up
  • Coordinate referrals and warm handoffs to behavioral health, medical, and social services
  • Conduct follow‑up within 24‑72 hours and monitor engagement for up to 30 days
  • Address barriers to care and re‑engage individuals as needed
  • Supervise and support Peer Specialists in service delivery and documentation
  • Participate in case reviews, team meetings, and multidisciplinary coordination
  • Serve as liaison between clients, families, and providers
Program & System Support
  • Maintain accurate documentation and service tracking
  • Support quality improvement, data reporting, and program development
  • Participate in debriefings, trainings, and crisis response planning
Requirements

Education & Licensure

  • Master's Degree in Social Work or related field required
  • Active New Jersey Licensed LAC or LSW required

Experience

  • Minimum 3 years of experience in behavioral health, substance use, or co‑occurring disorders
  • Experience in crisis response, emergency services, or community‑based care preferred
  • Prior supervisory or team leadership experience preferred
  • Knowledge of crisis intervention, trauma‑informed care, and recovery‑oriented practices
  • Ability to provide clinical oversight and support multidisciplinary teams
  • Strong communication, documentation, and decision‑making skills
  • Ability to navigate behavioral health and community resource systems

Additional Requirements

  • Valid driver's license, insurance, and ability to operate agency vehicles
Physical Demands
  • Frequent travel within the community
  • Ability to sit/stand for extended periods
  • Ability to lift up to 50 pounds
