Qualifikationen

• Considerable knowledge of clinical treatment and social casework with children/adolescents in mental health and substance abuse settings.
• Knowledge of child/adolescent growth and development and range of treatment provided by the center.
• Bachelor's Degree preferred or Associate Degree with minimum two years' relevant experience.

Aufgaben

• Provide case management and support services to the Treatment Team to facilitate clinical management of assigned cases.
• Coordinate documentation, discharge planning, therapeutic passes, and related activities with the treatment team.
• Communicate with referral sources and other mental health professionals regarding residents' progress and treatment planning including discharge planning and aftercare.
