Job summary

The role includes direct supervision of staff, coordinating service delivery, budgeting, and medication administration oversight. A $2,500 hiring bonus is offered to qualified applicants.

Qualifications

• Designated Coordinator must meet one education/experience pathway per Minnesota Statute 245D.
• 4+ years of full-time direct care experience required if using the 50-hour training option.
• CPR/First Aid/Meds Administration certification may be obtained after hire.

Responsibilities

• Program Oversight: supervise one or more residential homes serving 4+ individuals.
• Service Delivery: ensure person-centered outcomes with health and safety.
• Team Leadership: supervise direct support professionals with coaching and evaluation.
• Compliance & Quality Assurance: monitor 245D requirements and policies.
• Collaboration: coordinate with clinicians, therapists and community partners.
• Direct Care: provide at least 16 hours of hands-on support; additional hours as needed.
• Staffing & Scheduling: manage team schedules and open shift coverage.
• Administrative Duties: manage budgets, documentation and regulatory communications.

Bachelor’s degree in human services + 1 year direct care

Associate degree in human services + 2 years direct care

Diploma in human services + 3 years direct care
