Full-Time | In-Office | Shared After-Hours On-Call Rotation

About the Role

Right at Home Southwest Houston is seeking a positive, resourceful, and highly organized Care Coordinator & Operations Specialist to support our daily operations.

This is a hands-on role for someone who enjoys working with people, thrives in a fast-paced environment, and takes full ownership of their responsibilities.

The ideal candidate is an exceptional communicator and natural problem-solver who anticipates challenges, builds productive relationships with caregivers, and finds solutions when unexpected situations arise.

Key Responsibilities

Caregiver Scheduling & Care Coordination (Primary Responsibility)

• Take complete ownership of caregiver scheduling, ensuring reliable coverage and continuity of care.

• Proactively anticipate staffing challenges and develop contingency plans for call-offs, emergencies, and unexpected schedule changes.

• Resolve last-minute scheduling issues independently, creatively, and efficiently.

• Build and maintain positive, productive relationships with caregivers to promote engagement, reliability, and retention.

• Coordinate caregiver assignments based on client needs, compatibility, availability, and preferences.

• Maintain clear, professional, and timely communication with caregivers, clients, and families.

• Provide direct client care as a last resort when no qualified caregiver is available, subject to appropriate training and authorization.

• Participate in a shared after-hours on-call rotation.

Office Operations & Caregiver Support

• Conduct caregiver orientations and assist with onboarding and training.

• Support day-to-day administrative activities, recordkeeping, and office coordination.

• Maintain accurate schedules, client records, and caregiver information using WellSky Personal Care.

• Assist with caregiver engagement and operational improvement initiatives.

• Support management with other operational priorities as needed.

Additional Responsibilities (As Time Permits)

• Conduct occasional client home visits to monitor satisfaction and strengthen relationships.

• Assist with marketing initiatives and community outreach activities.

• Visit local senior centers, community organizations, and referral partners when scheduling and operational responsibilities allow.

Qualifications

Required

• Minimum 2 years of experience in caregiver scheduling, care coordination, staffing, or related healthcare operations.

• Exceptional communication and relationship-building skills.

• Strong problem-solving abilities, resourcefulness, and the ability to remain calm under pressure.

• A proactive, ownership-driven mindset with the ability to anticipate challenges and plan ahead.

• Positive attitude, adaptability, and willingness to take initiative.

• Excellent organization, time management, and multitasking skills.

• Proficiency in Microsoft Office and scheduling.

• Valid driver's license and reliable transportation.

• Willingness to participate in after-hours on-call rotation and provide occasional direct care coverage when necessary.

Preferred

• Previous experience in a non-medical home care agency.

• Experience with WellSky Personal Care.

• Bilingual in English and Spanish.

• Residence within or near our service territory, including Rosenberg, Richmond, Fulshear, Needville, and surrounding communities.
