Job summary

AccessHealth in Wisconsin is seeking a dedicated role to serve as the primary contact for families of children with developmental, physical, or severe emotional disabilities, guiding access to community services through assessment, planning, coordination, and ongoing support.

You will conduct functional assessments, develop service plans with families and providers, authorize and monitor services, and document thoroughly.

Qualifications

  • Bachelor’s degree in a health or human services field.
  • At least 1 year of experience working with children in the target population.
  • Strong communication, documentation, and organizational skills.
  • Proficiency with electronic health records and computer applications.
  • Valid driver’s license and reliable transportation.
  • Bilingual fluency in Spanish is preferred but not required.

Responsibilities

  • Utilize evidence-based practices supported by organizational and funding guidelines.
  • Accurately complete time reporting.
