Preferred Home Health Care & Nursing Services seeks a Care Coordinator to support children with developmental, physical, or emotional disabilities through the CLTS Waiver Program.

The role involves serving as the primary contact for families, conducting assessments, developing and authorizing individualized service plans, coordinating services, and leading family team meetings. Travel and crisis response are expected as part of daily responsibilities.

Qualifications

• Bachelor's degree in health or human services or related field.
• At least 1 year of experience with children in similar populations.
• Proficiency with documentation and electronic health records.

Role Overview

Serve as the primary contact for families, assisting in accessing services that promote safety and well-being. Responsibilities include conducting assessments, developing and authorizing individualized service plans, coordinating services, facilitating family team meetings, maintaining accurate documentation, collaborating with service teams, and participating in staff training.

• Proficiency with electronic health records and computer applications
• Valid driver's license, reliable transportation, and good driving record
• Bilingual Spanish skills are preferred but not required
