Job summary

Nascentia Health is seeking a qualified care coordination support professional to assist the Care Coordinator and Authorization Coordinator in delivering patient-centered care. You will help integrate technology for member engagement, train staff, cover responsibilities during absences, and monitor call queues to maintain staffing and timely responses.

Qualifications

• Requires 3-5 years’ experience in healthcare or related field.
• Proven analytical and problem-solving skills.
• Excellent communication, verbal and written.

Responsibilities

• Supports the Care Coordinator and Authorization Coordinator with team-based interventions.
• Supports integration of technology as a method of member engagement.
• Provides ongoing training, support and mentoring to staff and onboarding.
• Manages call queue to ensure adequate staffing for timely calls.
• Assists with CQI activities and quality audits to meet team responsibilities.
• Reviews EMR and other software reports and ensures accurate, timely documentation.
