Job summary

MedCentris is seeking a dedicated Nursing/Care Assistant to support patient care delivery under licensed supervision. You will collect intake information, coordinate home visits, and maintain accurate EMR records while ensuring compliance with HIPAA and company policies.

The role involves scheduling, inventory management, and collaboration with discharge planners, facility contacts, and the business development team to ensure timely patient access and satisfaction.

Qualifications

• One-year experience in a medical office or health care setting preferred.
• Certification as a CNA, CMA preferred.
• Ability to communicate verbally and in writing effectively.
• Demonstrates proven decision-making skills.
• Must read, write, and comprehend English
• High School Diploma or equivalent required

Responsibilities

• Promote and exemplify Company mission, vision, and values.
• Receive intake information and enter it in the computer.
• Schedule and maintain patient schedules.
• Order home visit supplies and manage inventory.
• Verify patient contact information and scheduled visits.
• Coordinate with business development, discharge planners, and facility contacts.
• Comply with HIPAA regulations and compliance programs.
• Maintain case notes and confidentiality of patient records.
• Report to work on time and complete tasks as directed.
• Represent MedCentris professionally.
• Assist with appointment confirmations and patient status reports.
• Coordinate care with home health agencies.
• Communicate effectively with clients, families, and health care professionals.
• Participate in in-service programs to maintain competency.
• Collect and record relevant patient health data.
• Develop patient care plans and document in EMR.
• Execute other duties as assigned
