Job summary

Nascentia Health is seeking a Care Coordinator to support patient-centered care, coordinating authorizations, scheduling, and communications. This onsite role runs Monday–Friday, 8:00 AM–4:30 PM and works closely with Care Managers and clinical teams.

You will handle authorization data, assist with Member services, and participate in CQI initiatives to improve outcomes. A strong admin and healthcare IT background is preferred.

Qualifications

  • High school diploma required; Associate’s degree in a related field preferred.
  • Two or more years of healthcare experience or completion of a Medical Assistant program.
  • Strong computer literacy and ability to learn new systems.
  • Proficiency in Microsoft Office and healthcare software.
  • Strong analytical and problem-solving skills.
  • Ability to work independently while collaborating effectively with a team.
  • Excellent verbal and written communication skills.
  • Demonstrated discretion in handling confidential information.
  • Ability to adapt to changing priorities, deadlines, and workflows.

Responsibilities

  • Review, process, and enter authorization data into computerized systems.
  • Monitor expiring authorization reports to ensure continuity of services.
  • Assist Care Managers with scheduling and coordinating Member services.
  • Collaborate with clinical team members and participate in team meetings.
  • Support scheduling and tracking of Member contacts, visits, and assessments.
  • Answer and triage phone calls; escalate issues when needed.
  • Communicate with Members to support care coordination needs.
  • Perform clerical tasks such as filing, copying, faxing, typing, and message-taking.
  • Participate in CQI initiatives and continuous quality improvement.
  • Perform other duties as assigned.

As a Care Coordinator, you play a vital role in supporting patient care by ensuring services are authorized, scheduled, and coordinated efficiently. Working closely with Care Managers, clinical teams, and community agencies, you help keep care plans on track and patients supported often behind the scenes, but always essential.

About the Role

The Care Coordinator provides administrative and technical support to the Care Coordinator Supervisor and interdisciplinary team, including the members. This role focuses on the operational aspects of care coordination, including telephonic communication and coordination of services, authorization processing, service coordination, and ongoing communication with Members and care partners.

This position is onsite at a Nascentia Health office, Monday–Friday, 8:00 AM–4:30 PM.

What You’ll Do

Support efficient, accurate care coordination through strong organizational and communication skills.

Key responsibilities include:
  • Review, process, and enter authorization data accurately into computerized systems
  • Monitor and manage expiring authorization reports to ensure continuity of services
  • Assist Care Managers with scheduling and coordinating Member services as directed
  • Collaborate regularly with clinical team members and participate in team meetings
  • Act as a resource to Care Managers, supporting scheduling and tracking of Member contacts, visits, and UAS assessments
  • Answer and triage phone calls, respond to questions, and **escalate** issues as appropriate
  • Communicate with Members on a regular basis to support care coordination needs
  • Perform clerical and administrative tasks including filing, copying, faxing, typing, and message‑taking
  • Participate in performance improvement and continuous quality improvement (CQI) initiatives
  • Perform other duties as assigned

Physical Requirements:
  • Ability to sit or stand at a desk 90% of the day. Frequent sitting, standing, walking.
  • Visual/hearing ability sufficient to comprehend written or verbal communication.
  • Ability to express oneself verbally and in writing.
  • Must be able to engage in real‑time phone communication with clients, caregivers, and providers as a core job function.
  • Extended periods of computer use.
  • Ability to lift and move objects up to 40 pounds.
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
