Job summary

UniversaCare is seeking a Care Coordinator Team Lead in New York to supervise scheduling staff, support coordinators, and ensure high-quality service delivery. You will partner with operations leadership to optimize staffing coverage and workflow, while promoting accountability and client satisfaction.

The ideal candidate has 2+ years in home care coordination or healthcare operations, strong Excel skills, and experience with scheduling platforms such as HHA Exchange or AxisCare.

Qualifications

• Minimum 2 years in home care coordination or healthcare operations.
• Prior supervisory or team lead experience preferred.
• Experience with HHA Exchange and AxisCare required or strongly preferred.
• Strong Excel skills for reporting and data tracking.
• Excellent communication and organizational skills.
• Ability to work in a fast-paced, high-volume environment.
• Bilingual in English and Spanish a plus.

Responsibilities

• Supervise and support care coordinators and scheduling staff.
• Monitor productivity, attendance, and daily workflow.
• Guide escalated scheduling and staffing concerns.
• Onboard and train new coordinators.
• Promote accountability and efficiency within the department.
• Oversee caregiver scheduling, shift coverage, and case coordination.
• Coordinate with Recruitment and HR on staffing requirements.
• Ensure timely communication with caregivers, clients, families.
• Support continuity of care and service quality standards.
• Participate in weekend on-call rotation.
• Use HHA Exchange and AxisCare for scheduling, tracking, reporting.
• Maintain documentation and records.
• Generate reports and analyze trends with Excel.
• Assist leadership with operational tracking and performance reporting.
• Ensure compliance with policies and home care regulations.
• Resolve client and caregiver concerns professionally.
• Maintain positive relationships with field staff and clients.
• Support caregiver retention and engagement.
• Ensure excellent customer service standards.
