Job summary

Bronx Community Health Network seeks a CHW Supervisor to oversee Community Health Workers, coordinate care, and promote health equity in under-resourced communities. You will lead training, supervise staff, and drive performance improvements within a patient-centered, team-based care model.

The role requires building partnerships with clinical teams and community organizations, maintaining EHR documentation, and ensuring compliance with HIPAA and HRSA requirements.

Qualifications

• Bachelor’s degree in health education, public health, community health, or related field.
• Minimum of three (3) years in community health, care coordination, population health, case management, social services, patient navigation, or related field.
• Minimum of two (2) years of supervisory or team leadership experience, preferably supervising CHWs or other community-based staff.

Responsibilities

• Supervise, coach, and evaluate CHWs and related staff.
• Conduct regular staff meetings, supervision sessions, and performance evaluations; provide feedback and support performance improvement.
• Develop staffing schedules, assign caseloads, and ensure coverage and workload distribution.
• Monitor CHW productivity, caseloads, service quality, documentation, and program metrics; use data to identify opportunities for improvement.
• Promote teamwork, accountability, patient-centered care, and continuous quality improvement.
• Oversee outreach and patient engagement activities to connect patients to primary care and resources.
• Maintain knowledge of community resources and strengthen referral pathways; ensure HIPAA compliance.
