Job summary

Red Rock Behavioral Health Services is seeking an Advanced Care Coordinator Level I in Watonga, OK. The role focuses on coordinating medically necessary services, advocating for families, and delivering trauma-informed, culturally sensitive care within a wraparound framework.

The candidate will assess needs, coordinate mental and physical health services, and collaborate with the health team to implement care plans while maintaining compliance and confidentiality.

Qualifications

• BHCM II certification required
• Alternative qualifications include behavioral health degree or RN with BH experience or any degree with BH experience
• Able to exercise independent judgment with diverse populations
• Complete in-service and external trainings
• Must have basic computer skills
• Two+ years of experience with community services and health agencies and cultural diversity
• Committed to the mission of Red Rock BHS
• Valid Oklahoma driver’s license and insurance
• Ability to adjust schedule to client needs

Responsibilities

• Demonstrates ability to work with diverse populations
• Coordinates services including mental and physical health
• Completes multi-level consumer and family assessments
• Communicates with the community team and review committee
• Provides advocacy and linkage to community resources
• Provides crisis assistance
• Maintains relationships with consumers and community organizations
• Promotes PTSD and mental health awareness
• Assists with Care Plans within Red Rock Health Team
