Red Rock Behavioral Health Services in Hobart, Oklahoma seeks an Advanced Care Coordinator Level I to join the Health Team. You will serve as the primary coordinator, ensuring access to medically necessary services, coordinating supports, and delivering care in a strengths-based, family/youth-driven approach.

The role emphasizes collaboration with community resources, crisis assistance, documentation, and participation in trainings to maintain high-quality care for diverse populations.

Qualifications

• BHCM II certification or equivalent credentials.
• Bachelor’s or Master’s degree in Behavioral Health.
• RN with Behavioral Health experience.
• Bachelor’s or Master’s degree in any field plus 6 months BH experience.
• Strong communication and organizational skills.

Responsibilities

• Coordinates services including mental and physical health.
• Completes multi-level assessment of consumer and family.
• Maintains relationships with community teams, review committees and other groups.
• Provides advocacy and linkage to community resources.
• Provides crisis assistance.
• Completes required Health Team and agency trainings.
• Maintains core competencies and participates in meetings and trainings.
