Job summary

Red Rock Behavioral Health Services is seeking a Care Manager to implement comprehensive care plans, coordinate services, and support individuals with mental health needs in Shawnee, OK. The role involves collaboration with the Health Team, treatment providers, and families to achieve clinical outcomes.

The position emphasizes crisis intervention, case management, and ongoing assessment, with responsibilities including documentation, referrals, and maintaining privacy across settings.

Qualifications

• Certified BHCM I (BHCM I) required.
• High School Diploma or equivalent.
• Bilingual preferred.

Responsibilities

• Provides advocacy, coordination, side-by-side support, problem solving, training, and supervision to help consumers obtain daily living necessities including medical and dental health care, legal and advocacy services, and entitlements.
• Participates in disposition/treatment team meetings and contributes relevant treatment information.
• Assists in developing and revising overall Service Plan goals and objectives.
• Coordinates services with DHS and other sources; advocates to ensure plans are followed, and arranges follow-up support.
• Educates and supports consumers’ families.
• Delivers direct clinical services to consumers in office and community settings and teaches symptom-management techniques.
• Documents progress and maintains permanent records; completes paperwork and prior authorizations timely.
• Ensures confidentiality and informs consumers of rights; supports co-workers to cover during absences.
• Maintains knowledge of Medicaid, ODMHSAS, CARF requirements and funding sources.
