Job summary

Red Rock Behavioral Health Services is seeking an Advanced Care Coordinator Level I in Hobart, OK. You will serve as a single point of accountability to ensure access to medically necessary services, coordinate supports, and deliver strengths-based care within a health team, including wraparound services as needed.

The role requires BHCM II certification (or equivalent) or relevant RN/Behavioral Health credentials, with flexibility to serve diverse populations and travel locally as required.

Qualifications

• BHCM II certification or equivalent required.
• Bachelor's or Master’s in Behavioral Health or RN with BH experience.
• Must complete in-service and external trainings.
• Two+ years in community services with diverse populations.

Responsibilities

• Coordinates services including mental and physical health.
• Completes multi-level consumer/family assessments within timelines.
• Communicates with community team and review committee.
• Provides advocacy and linkage to community resources.
• Provides crisis assistance and safety planning.
• Attends meetings, trainings, and workshops.
