Job summary

Red Rock Behavioral Health Services in Chandler, OK is seeking a Care Manager I to implement comprehensive care plans across mental health, physical health, and life goals. You will work with the Health Team, treat individuals with mental health or substance use needs, and coordinate services with DHS and referral sources.

Responsibilities include crisis intervention, case management, treatment planning, client advocacy, and education.

Qualifications

• General skill in exercising independent judgment, tact, patience, and understanding; maintaining effective working relationships and strategic alliances with consumers, other staff, the community and other non-profit organizations
• Ability to complete in-service and external training requirements
• Valid Oklahoma Driver License, a clean Motor Vehicle Record, and current Auto Insurance

Responsibilities

• Provides practical help and advocacy to help consumers obtain daily living necessities, including medical and dental health care and transportation
• Participates in disposition/treatment team meetings with pertinent information for treatment
• Assists in developing and evaluating overall Service Plan goals and objectives
• Coordinates approved services and communicates with DHS or other referral sources; advocates on behalf of consumers
• Educates and supports consumers' families
• Provides direct clinical services to consumers in office and community settings to teach behavioral symptom-management techniques
• Coordinates with outside inpatient services to detoxify consumers and link to self-help programs
• Builds rapport with relevant local, state, and federal government agencies
• Promotes community education projects and supports care plans for Red Rock Health Team
