Greater Family Health in Hoffman Estates, IL is seeking a Full-Time Care Coordinator to facilitate coordination of care and continuity of care for third-party managed care patients. Located in the Chicagoland area, the organization offers comprehensive medical, dental and behavioral health services across multiple sites.

The role involves facilitating referrals, follow-ups, prior authorizations, and coordination of care in line with clinical measures and care plans.

Qualifications

• High School Diploma or GED required.
• Associate or bachelor’s degree in health or social services preferred or 3 years health care experience.
• Medical Assistant, CNA, or Community Health Worker experience preferred.
• Bilingual in Spanish is highly preferred but not required.

Responsibilities

• Facilitates referrals for specialty care and testing for applicable third-party managed care patients.
• Completes referral follow ups for specialty care and testing.
• Assists patients in obtaining prior authorization for diagnostic testing, equipment or other services.
• Coordinates care to align with patients' clinical measures and care plans.
• Performs health risk and social determinants of health assessments and screenings.
• Follows up with patients after ER visits and ensures timely follow up with providers.
• Assists patients with adherence to self-management goals in coordination with clinical staff.
• Adheres to quality assurance and risk management standards.
• Reports to all scheduled work sites and shifts on time; locations may change at the discretion of the supervisor.
• Participates in training and supervision; implements directives from supervisors.
• Performs other duties as assigned.
