Responsibilities

• Prioritize workload with the care management team to address high-priority tasks promptly.
• Assist clinicians in delegating tasks and coordinating payer communications for approvals.
• Engage in patient care transitions and assist with scheduling appointments and transportation as needed.
• Help patients navigate the health care system, explain insurance coverage, and locate providers.
• Support procurement of services/equipment per department protocols.
• Facilitate documentation for care transitions (FL-2, PASRR) accurately and timely.
• Maintain effective communication and escalate issues per guidelines with the care team.
