The Care Manager will guide safe discharge, ensure effective care transitions, and advocate for patients throughout their healthcare journey. The schedule features 40 hrs/week on five 8-hour shifts, with weekend and holiday rotation, no nights, and no on-call.

Qualifications

• RN inpatient experience in acute care (≥2 years).
• Care management interest and drive to direct safe discharge.
• Advocacy and leadership in patient care progression.
• Desire to join supportive UNC Health team in Chapel Hill.
• Ability to work in a multidisciplinary team.

Responsibilities

• Conduct comprehensive care manager assessments using standardized tools.
• Develop individualized care plans with providers, patients and families.
• Identify barriers to care and implement interventions.
• Coordinate tasks and interventions to support care plans.
• Collaborate with providers, community resources and payers.
• Participate in quality improvement initiatives to meet goals.
• Coordinate and facilitate care transitions across settings.
• Document and bill for services per regulatory requirements.
