Job summary

UNC Health Care in Chapel Hill is seeking a Clinical Administrative Nursing Care Coordinator to support patient care areas. You will coordinate care, discharge planning, data analytics, and patient education while collaborating with an interdisciplinary team.

The role requires RN licensure in NC and at least five years of nursing experience. This is a full-time onsite position with a 40-hour work week and a 56,000-plus teammate organization.

Qualifications

• Graduation from a state-accredited school of professional nursing.
• Licensed to practice as a Registered Nurse in the state of North Carolina.
• Five (5) years of professional nursing experience.

Responsibilities

• Assesses patient status of a specialized population and coordinates care with the interdisciplinary team.
• Provides inservices and presentations to nursing, medical and ancillary staff on specialized topics.
• Collects and analyzes data for trends, benchmarking, and quality improvement.
• Coordinates post-hospitalization care and discharge planning with clinics, home care, and other providers.
• Promotes ongoing professional development and adherence to nursing standards.
