Job summary

PT Associates is seeking a Care Coordinator to serve as the central contact for patients, providers, and clinical staff. You will schedule appointments, communicate treatment plans, and support patients throughout their care journey to promote positive experiences and outcomes.

You will coordinate referrals, verify documents, maintain EMR data, and collaborate with the care team to remove barriers. A strong emphasis on organization, communication, and patient advocacy pervades the role.

Qualifications

• High school diploma or equivalent required.
• 1–3 years in healthcare coordination or related role preferred.
• Knowledge of medical terminology and healthcare workflows preferred.
• Experience using EMR systems.
• Typing speed 45 WPM; proficiency in Word/Excel.

Responsibilities

• Coordinate and schedule patient appointments, evaluations, procedures, and follow-ups.
• Communicate treatment plans, appointment details, and care instructions to patients and caregivers.
• Serve as a liaison between patients, providers, and care team to facilitate communication.
• Monitor and track patient progress and follow-up care.
• Maintain accurate patient information in EMR.
• Respond to patient inquiries promptly and professionally.
• Coordinate referrals to specialists and services as needed.
• Verify authorizations and supporting documentation prior to services.
• Assist patients in understanding care plans and navigating care system.
• Collaborate with teams to identify and resolve barriers to care.
