Qualifications

• Graduate of an accredited program: LPN/LVN or RN.
• MSW with licensure or BSW with licensure per state.
• Minimum 1 year in healthcare; case management or discharge planning a plus.
• Excellent interpersonal and written communication skills.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Refer high-risk patients for additional support as needed.
• Serve as patient advocate and support discharge planning.
• Participate in interdisciplinary rounds and care conferences.
