Job summary

ScionHealth is seeking a licensed professional to coordinate patient care across the continuum at Kindred Hospital Paramount and ARU. You will collaborate with the Interdisciplinary Care Transitions team to ensure optimal resource use, care delivery, and discharge planning.

The role requires clinical and psychosocial coordination, knowledge of payor practices, and strong communication. A health care license (LPN/LVN or RN) or MSW/BSW with licensure is required, with 1 year of healthcare

Qualifications

• Graduate of an accredited program required: LPN/LVN or RN.
• Master of Social Work with licensure or Bachelor of Social Work with licensure.
• One year of experience in healthcare setting.
• Experience in case management, utilization review, or discharge planning a plus.
• Healthcare professional licensure required as LPN/LVN, RN, or LCSW or LSW if required by state regulations.

Responsibilities

• Assist in coordinating clinical and/or psycho-social activities with the Interdisciplinary Team and Physicians.
• Assist with effective care coordination and efficient care facilitation.
• Remain current on reimbursement modalities, community resources, case management, psychosocial and legal issues affecting patients and providers.
• Refer high risk patients who would benefit from additional support.
• Serve as a patient advocate.
• Participate in interdisciplinary patient care rounds and conferences.
• Collaborate with clinical staff in the execution of the plan of care and achievement of goals.
