Qualifications

• Must read, write and speak fluent English.
• 2+ years experience in healthcare setting (Required).
• Knowledge of government and non-government payor practices and reimbursement.

Responsibilities

• Coordinate care with Interdisciplinary Team and Physicians.
• Monitor patient stays for effective care coordination.
• Refer high-risk patients for additional support.
• Serve as patient advocate to optimize outcomes and costs.
• Participate in interdisciplinary rounds to review goals and resources.
• Coordinate with payors and post-acute providers across the care continuum.
