CenterWell Home Health is seeking a Medical Social Worker to join an interdisciplinary team delivering in-home care to patients. You will assess social and emotional needs, develop care plans, and coordinate with family and health professionals.

Supervising a Social Worker Assistant, you will ensure compliant, patient-centered services. The role requires licensure if mandated, CPR certification, a valid driver’s license, and local travel.

Competenze

• Minimum of one year of experience as a social worker in healthcare, home health, or hospice.
• Social Work licensure in the state of practice, if required by law.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience with discharge planning and community resources.
• Ability to organize tasks, set priorities, and work under pressure.
• Fluent English (reading, writing and speaking).

• Assess the patient's social and emotional state as it relates to their illness or injury, care needs and response to treatment.
• Assess relationships between medical/nursing needs, finances, and community resources.
• Provide actions to obtain community resources to support recovery.
• Communicate with patients, families, and organization colleagues.
• Provide ongoing assessment of patient and family needs and responses to treatment.
• Participate as a member of the interdisciplinary care team in coordination activities.
• Evaluate the Social Work Assistant to ensure services meet standards.
• Maintain and submit documentation including case conferences, visit reports, and notes.
