Job summary

Baptist Health in Boynton Beach is seeking a Social Work Case Manager to intervene with patients and families for psychosocial needs, provide crisis intervention, and coordinate discharge planning. The role requires a Master’s in Social Work and experience in hospital settings.

You will work with a multidisciplinary team to ensure smooth transitions and connect patients with community resources. Compensation includes a base salary with an annual range reflecting experience, and a $10,000 bonus

Qualifications

• Excellent interpersonal communication and negotiation skills.
• Strong analytical, data management and computer skills.
• Ability to work independently and exercise sound judgment.
• 0-1 years of social work experience in hospital or related health care setting, Emergency Department experience preferred.
• Internship will be considered as experience.

Responsibilities

• Intervenes with patients/families for psychosocial needs and provides crisis intervention.
• Coordinates and facilitates development of a discharge plan of care.
• Serves as liaison between patients and community resources.
• Identifies psychosocial and environmental needs related to admission, diagnosis, treatment and discharge.
• Works with the interdisciplinary team to implement a comprehensive discharge plan.
