Qualifikationen

• Master’s degree required; MSW preferred for the role.
• 0–1 year hospital social work experience; ED experience preferred.
• Excellent interpersonal communication and negotiation skills.
• Strong analytical, data management and computer skills.
• Knowledge of discharge planning, crisis intervention and case management.

Aufgaben

• Intervene with patients and families for psychosocial needs and support.
• Coordinate discharge planning and provide referrals to community resources.
• Act as liaison between patients, families and healthcare team.
• Assess psychosocial needs across neonatal to adult patients.
• Provide crisis intervention and bereavement support as needed.
