The role requires a Master of Social Work (MSW) and emphasizes collaboration with the interdisciplinary team to develop and implement comprehensive discharge plans for all patient groups.

Qualifications

• Masters in Social Work (MSW).
• MSW required for continued non-leadership role if prior Social Work licensure exists.
• Ability to solve problems in a fast-paced hospital setting.
• 0-1 years of hospital social work experience; Emergency Department preferred.
• Internship counts as experience.

Responsibilities

• Intervenes with patients/families for psychosocial needs and support in coping with illness.
• Provides crisis intervention and coordinates discharge planning.
• Educates, counsels and advocates for patients and families.
• Provides referrals to community services and coordinates with healthcare team.
• Identifies psychosocial and environmental needs related to admission, diagnosis, and discharge.
• Acts as liaison between patients and community resources; may provide inpatient emotional support.
• Assesses abuse/neglect situations and collaborates on safety planning.
• Works with the interdisciplinary team to develop comprehensive discharge plans.
