Job summary

Baptist Health in Florida is seeking a Social Work Case Manager 2 to support patients and families with psychosocial needs, coordinating education, discharge planning, and referrals.

You will collaborate with a multidisciplinary team, provide crisis intervention and bereavement services, and help navigate community resources. Requires MSW and LCSW eligibility; hospital SWR experience preferred.

Qualifications

• Master’s degree in social work is required
• LCSW or eligibility to sit for the LCSW exam within 12 months of entry into SWR 2
• Minimum 3 years hospital social work experience
• Excellent interpersonal communication and negotiation skills
• Strong analytical, data management and computer skills

Responsibilities

• Intervenes with patients/families with psychosocial needs and coordinates discharge planning
• Coordinates with the interdisciplinary team to develop and implement a comprehensive discharge plan
• Provides crisis intervention and bereavement services
• Identifies psychosocial needs and makes appropriate referrals to community resources
