Minimum qualifications include MSW; 0-1 years of hospital social work experience and internship considered as experience. Excellent communication and data skills are required.

Qualifications

• Demonstrate ability to solve problems in a fast-paced environment.
• 0-1 years of social work experience in hospital or related health care setting, Emergency Department experience preferred.
• Internship will be considered as experience.
• Excellent interpersonal communication and negotiation skills.
• Strong analytical, data management and computer skills.

Responsabilités

• Intervenes with patients/families for psychosocial needs and coordinates discharge planning.
• Provides crisis intervention and patient/family services via case finding referrals.
• Acts as liaison between patients and community resources and other care team members.
• Identifies psychosocial and environmental needs related to admission, treatment and discharge.
• Supports development and implementation of a comprehensive discharge plan.
