Qualifications

• Master of Social Work required.

• 3 years of experience as a Social Work Case Manager required, preferably in an acute care setting.
• Qualified Supervisor status to supervise MSW interns for LCSW licensure preferred.

Responsibilities

• Facilitates the safe movement of patients through the continuum of care.
• Represents department as member of a specialized team and in professional organizations.
• Facilitates group meetings and education.
• Acts as a department liaison with community social and health services.
